Also possibly Darkmesh Helm of War and Well-Worn Ivory Beads (TMap8) and technically Underhill Soldier Crest (lowbie quest). These are the results of checking the wiki. These items may have changed since they were added there (I couldn't find them on fomelo.) There should probably be a Defense+ item list added onto the Defense page I suppose.
