how do you creat a guild?

I was wondering how I go about making a guild. I've been reading through the forums and heard a billion different answers to this so I thought I'd start a thread and see if someone could help. Thanks for your help!

I thought it had been dropped to 5 or 6 due to size of the server. Had been some mention of it in the Afterlife guild thread, but it is gone now.

Because it was changed back to 10 shortly after that.
