Yellow Lines

Everytime i press enter on the server selection screen I get yellow lines on the top of the screen and a locked up computer, Whats causing this and is there a fix? ((I am a Patch Happy person))
 
go to start run and type DXDIAG then hit ok

there is an option there to save the results

copy and paste the results here
