Shaman Medicine Bag

Hello all,

I am not sure if I should petition in game or hopefully get some info on the boards. I am a Lvl 56 Ogre Shaman with Alchemy skill of 85. I recently logged on to find my Medicine bag missing. I went to Oasis and got another Mukewater Croc Hide, but I cant seem to find the quest NPC now in Oggok. I thought last time I gave it to my guild master, but nothing is working. Can someone please help.
 
We have had various quest items going randomly missing as well. Items we *know* we had, that weren't there when we went to do the turn in.

Is this becoming a bigger issue in general?
