These are the number one petition issue. It's not something we can fix, but from what I've noticed it happens when you switch items really frequently and the client/server seem to get out of synch with what is there. Mod rods are pretty notorious for this, but so are clickies of any kind. I don't mean to sound preachy, but if you've had this happen to you, you might want to consider leaving yourself a spot or two for clicky items so you don't have to pick up a bag, right click an item, and put the bag back, which is simply risky behavior with this bug going around.

Like I said, this is simply what I noticed, and I don't mean to say it WILL cause it to happen, but it seems MORE LIKELY to happen when you engage in such behavior. I issued a similar warning about soulbond/permanent Aego and your hp a long time ago, and I'm just doing the same here.
 
What i usualy do is keep 2 basic inventory slots free for clickies. 1 slot is for my scarerow clicky and the other is usually free for modrods during raids. Or, for SK/DHK necks which are in a bag.

I did have it once tho that my DHK poofed after i took it from my bag, switched it with my modrod and clicked it. But i guess that was just bad luck because i just happened to have a modrod at that time.
 
It has happened to me when switching out my weapons for an instrument. I was scared shitless but thankfully the GMs were on top of it...well all the gms but rabb....hate that guy!
 
I also noticed if your in the middle ofa lag spike, and move something around, it automatically goes invisible in your inventory.... not sure if this is filed under *too obvious to post* but I for one take my hands away from the keyboard the moment i see my bar turn yellow
 
had this happen to me 3 times.. first time was an emerald scaled cloak .. got it fixed thank you gm-jael.. today i lost a tmap and a bag to this bug ........ currently waiting for a gm to fix
 
I've only seen this happen twice and it was both when rapidly moving materials in mass combining and thats over about 3 months, 1st time it was just some charcoal but 2nd was firewater which yally restored promptly.
 
It's happened to me once or twice but both times I was holding the item I lost while zoning (port neck swap out) which probably has something to do with it. Really I suppose zoning with it on my cursor is pretty stupid in the first place but just something to keep in mind.

Though today I had a really bizzare error. I forraged what appeared to be a leather tunic but when I went to put it in my inventory I got this error



and the tunic dissapeared. I thought it was odd and went on my way. But then when I logged on tonight I was wearing a mandrake root in my chest slot. My regular tunic was gone. I guess I'll be petitioning soon but I was curious if anyone has had anything close to this happen before. God, the gms probably hate me now.

Mandrake root by the way does not have good stats.
 
I've recoded this so that instead of deleting when there's an item consistency, it just forces a resend of what items are on the affected slots.
 
encountered a mini bug somewhat kinda sort of similar to this yesterday when I was rezzed while exiting a merchants inventory, upon zoning in from rez, all items i had, inventory and everything were gone. Stats also were naked stats. However camping and relogging fixed the problem.
