Weapon proc rates

For spells like the following (not procs on weapons)

Is the proc rate known or listed somewhere? Is it normalized for weapon speed or do faster weapons scale better with these procs?

I can think of a few buff procs I'd like to know the proc rate on, if anyone who knows is willing to share

Maul - Form of the River Keeper (shaman self buff)
Power of Althuna - Champion of Althuna (paladin self buff)
Divine Might Effect - Divine Might (paladin self buff)
Circle of Healing - Althuna's Conduit (paladin self buff)
Strike of Torment - Painstone Coronet (torment helm clicky buff)
Shadow Drain - Visage of the Frilakh Master (fearstone keep mask click buff)
LifetapEffectNormal - Melwin's Idol of Joy (melc click buff)

Buff procs all have the same chance, or did until very recently. Pretty sure only a couple of the newest things have adjusted rates.

(Delay * 3) / 20 = Chance out of 100 (decimals count) = 1.5% per second of delay, before haste/slow. Only happens on main-hand rounds, and only once, before the first swing. Worth noting that buff procs don't care if you miss, unlike item-based procs.

Just one more question: Based on your post, proc buffs CANNOT fire on slam, bash, kick, backstab, dragon punch, tail rake, etc., correct?

Right.

Except for bow proc buffs, which have a different formula.

Does the monk class tome Elemental Fists work this way as well? I ask because most of the time, I'll see both the fire and cold DDs at the same time but every now and then only see one or the other which seems odd.
